The John Lewis Middle School Health Office assists your student in accessing learning. We appreciate your assistance with this by:

• Ensuring your student(s) meet State of Illinois and Waukegan Public School District 60 mandated health requirements (immunizations and physical examinations) prior to the first day of school entry.

• Reminding your student(s): if your student is ill, the student must be seen in the Health Office prior to contacting you, the parent. 

• Please tell your student to go to the Health Office and see the Nurse if they are not feeling well. 

• Maintain working telephone numbers and current emergency contact names and numbers with the  Health Office.

• Students must be present in class to access academic content/learning.

Your Certified School Nurse at John Lewis Middle School is Mrs. Janet Ritter, BSN, RN, MHP, IL/PEL – CSN.  Mrs. Ritter is professionally trained, licensed and certified to assess, refer and treat/administer medications when prescribed by a physician.   Mrs. Ritter is a member of the National Association of School Nurses and receives school health updates through this professional association as well as communications from the Illinois Association of School Nurses, Illinois Department of Public Health, Illinois State Board of Education, American Academy of Pediatrics and other sources. Mrs. Ritter supports the Whole School, Whole Community, Whole Child Model by providing first aid, emergency care, and assessment of and planning for chronic conditions. In addition, health services provide wellness promotion, referral to prevention services (such as HIV testing), and student and parent education and resources. As appropriate, she assists with access/referrals to outside health care providers.

Mandatory Health Requirements for ALL 6th Grade Students and ALL Students New to Waukegan Public School District 60!

According to The School Code of Illinois, physical examinations and immunizations are required for all students prior to initial entrance into school and upon entrance into kindergarten, sixth and ninth grades. Below are the detailed requirements required to enter school.

State of Illinois Certificate of Child Health Examination:  Students are required to have a State of Illinois Certificate of Child Health Examination on file.  The Health Examination must be completed by October 15, 2019 for students to remain in school after October 15, 2019. Sports physicals do not meet the State’s requirement unless the examination is recorded on the State of Illinois Certificate of Child Health Examination form. 

State of Illinois Proof of School Dental Examination Form : Students are required to have a dental examination on file.  School dental examinations must have been completed within 18 months prior to the May 15, 2020 deadline (State of Illinois Proof of School Dental Examination Form completed by a dentist between December, 2018 and May 15, 2020).  

Complete immunization record signed by a doctor: 5 doses of DTap, 4 doses of Polio, 4 doses of Hib, 4 doses of Pneumococcal Conjugate, 3 doses of  Hepatitis B, 2 doses of MMR, 2 doses of Varicella or proof of the disease, Meningitis (MCV4) vaccine: one dose on or after the 11th birthday and one dose of Tdap (Tetanus, Diptheria, acellular Pertussis) are required for entry for 6th Grade and students new to Waukegan Public School District 60.

The School Code of Illinois recommends completion of  the State of Illinois Eye Examination by an optometrist/ophthalmologist prior to entering school for the first time and prior to entering 6th Grade.
